健康診断結果の配付について
Announcement: The distribution of medical checkup results
このことについて，４月実施の学生定期健康診断の結果を，
下記日程のとおり配付します。
We are distributing the medical checkup results of April as the following schedule.
健康診断を受診した学生は必ず取りに来てください。
Students who had medical checkup must go to receive the result.
学生証で本人確認したうえで配布します。
When you receive the results, student card is required for identification reasons.
１． 日時 ５月２６日（月）：１２：００～１３：００
５月２７日（火）：１２：００～１３：００
Date/Time ：May 26 (Mon) and 27(Tue) 12:00～13:00
２． 会場 ひばりラウンジ（福利施設 ２階）
Place ： Hibari Lounge (Dining Hall Building, 2F)
※「健康診断証明書」の発行ができます。
就職等で必要な場合は，証明書発行機で「健康診断証明書」を発行してください。
Health Certificate can be issued by certificate issuing machine outside of
the Academic Affairs Division. If you need a copy of your certificate, please use the machine.
